Disability Inclusion Action Plan Community Survey

About this survey 

Kyogle Council is developing a new Disability Inclusion Action Plan, or DIAP. The plan will guide practical action over the next four years to make Council services, information, places, employment and community life more accessible and inclusive. Council wants to hear directly from people with disability, carers, families, support people, service providers, businesses and community members across the Kyogle Local Government Area. The survey should take about 15-20 minutes. You can skip any question. You do not need an NDIS plan, a formal diagnosis or disability service support to take part. A family member, carer or support person can help you complete the survey. 


How your feedback will be used 

Kyogle Council and its project consultant, Spinal Life Australia / Sporting Wheelies, will review the responses to identify common themes, location-specific barriers and practical priorities for the new DIAP. Council directly manages some places and services. Other issues, such as public transport, health, housing and disability services, may require Council to work with other organisations or advocate to government. Please still tell us about these issues so the plan can identify the right role for Council. Responses will be reported as themes. Council will not publish your name or an identifiable personal story without your permission. Contact details provided for follow-up will be kept separate from your survey answers where practicable.
